How would I create different tiles which have the "!" on apoplexy?

Say if I wanted to change the look of the tile in the attachment, which is not usable (I think it's the skeleton tile, but in the palace), how would I do that? I cannot see it anywhere in the folder PR generated for VPALACE. Or, alternatively, can I change in the dungeon, but only have it change in the palace, or vica versa?

How would I copy all the gate files into the one skeleton left? Is it possible to just combine them and resize skeleton left?

Are you trying to make the skeleton look like a closed gate, like in Repetition of Time?

For that effect, you indeed need to combine the parts of the gate. The easiest way is to copy a whole gate from a screenshot.
You could copy the gate to either skeleton left or skeleton right, depending on where do you want the gate to appear within the skeleton tile.
In Repetition of Time, the images were changed as shown below:

That is, skeleton right became a gate, and skeleton left became a regular floor.
In RoT this tile is on the edge between two rooms, so the lack of a left railing is not apparent.
If you want to use it elsewhere then you should change skeleton left so it contains the left railing.
